New Entry Form

In case you haven't noticed, I added a fancy schmancy entry form for my giveaways. You can find it right below this post in the Proactive Parenting giveaway. I'm trying it out so we'll see how it goes. So far it seems like a good thing- it helps the person entering so that they don't have to leave multiple comments and all the info entered goes right into a spread sheet for my viewing pleasure!
